Should the flywheel be resurfaced when the clutch is replaced?

Look at it while the transmission is out. Glazed or scored means machine it or replace it, because a rough flywheel will make a brand-new clutch judder like the old one.

Do I need special tools for the installation?

A clutch alignment tool, plus a way to support the transmission on the way out and back in. It is one of the bigger jobs on these trucks, and there is no shame in handing it to a shop.

Can I keep driving on a slipping clutch?

For a while, but every slip is heat, and heat glazes the flywheel while it finishes off the disc. The sooner it goes in, the less you replace.
