Do I need an alignment after?

Yes. After front-end work, set the alignment so the truck tracks straight.

How do I know my front end needs this refresh?

With the front raised, rock the tire at 9 and 3 and 12 and 6 o clock and watch the joints and bushings for play. Wandering steering, clunks, and uneven tire wear mean the front end is tired.

Do I have to do the whole kit at once?

No, but doing it together restores the feel and saves repeat teardowns; at minimum do a full axle or joint pair so it stays even.
